Narrative

Operator was opening ports on screw conveyor (6CS34) to drain water. Operator saw some buildup of material in the east port and tried to knock it out with a 10"" crescent wrench. The screw caught the head of the wrench, jerking it out of operator's hand, cutting finger and thumb. Sutures were needed in index finger and thumb.
