## Narrative

Employee was bending a piece of 1/4"" x 2"" steel strap around a piece of pipe. He lodged one end of the strap between the pipe and a pipe hanger, and was heating the strap with a torch and hitting it with a hammer.  The strap came out of the area between the pipe and pipe hanger and struck the employee in the mouth.
