## Narrative

On 12/4/06, EE was operating the shuttle car, tramming from #5 entry to #5 left. EE turned the corner into #5 left and the shuttle car slid into the rib because of the grade of the bottom. The canopy of the shuttle car struck the rib, allowing a piece of coal to enter the operators car.
