## Narrative

Roof bolter was installing the second bolt of his first row when the roof inby the ATRS beam fell without warning.  A piece of rock flew up striking his leg above the knee. Cause of this injury was slickenside material falling without warning. Compliance with rules/regs was not a factor, mining equip/system not a factor, miner proficiency maybe a factor, personnel equip ok.
