Narrative

A 17'4"" x 3' x 1/2"" thick liner plate was being repositioned in the Pioneer feeder. The 5/8"" grade 8 bolt holding the upper right corner in place sheered allowing the plate to fall to the pan. The plate struck the employee on the left foot.
