A roof fall occurred in the 1 Main East in #1 entry in the intersection of cross-cut #141 measuring approximately 25ft. x 18ft. x 6ft. The area was supported with 6ft. tension rebar roof bolts. An undetected slip was determined to be the cause of the accident.
