A roof fall occurred int he #3 entry on the C1 connector Panel intersection spad #2918,5 crosscuts outby the section loading point. the fall was in an intersection measuring 20' x 20' x 5'. The area had been developed in November 2006 and was supported with 4' fully grouted bolts and had supplemental cribs set along the rib line on both sides.
