## Narrative

The employee was replacing bolts on the cone screen wear plate. He was using a 1/2"" ratchet with extension lying on the catwalk to access the bolt on the bottom side. The socket slipped off the bolt and the ratchet handle hit him in the nose resulting in a contusion.
