Narrative

On 1/21 employee walked across the parking lot at the mine lineout room when he slipped on the icy asphalt and fell to the ground. He initially said he was OK and refused medical treatment. He worked four 12-hour shifts, and had seven days off before calling off on 2/1 because his back hurt. We sent him for medical evaluation on 2/2 at which time he was put in a No Work status.
