Employees foot slipped while pulling up on a centrifuge to disassemble for cleaning. Foot hit the toeboard, jarring his right lower back/hip. Reported to his supervisor 2/7 as ""report only"". On 2/8, the employee reported back pain, so we took him to the doctor for evaluation. No drugs prescribed. Dr. recommended light duty on 2/8/07. Released to return to full duty 2/13/07.
