## Narrative

Employee was struck on the back of the head with a lead drill steel that was stuck in the roof. Cause of this injury was safety awareness and compliance with company rules. Rules was a factor, protective equipment not a factor, miner proficiency a factor, mining equipment/systems not a factor.
