A roof fall occurred on the 1st Main North belt at 37 1/2 crosscut (#3 Entry). Fall was 12' long, 12' wide and 6 ' high. Area was mined in Feb 1989. Fall occurred due to the deterioration of the immediate roff around existing roof bolts. Area was supported with 8' long expander shell bolts with a 2' resin anchor. Header blocks were used with the bolts. Cribs set along sides of belt
