Narrative

Employee stated he was on one knee removeing a cover on a poke hole (a metal cover approximately 1/4"" X 4"") on a 4"" round pipe. As he went to put the pin (1/4"" X 1"" X 6"" steel pin) back in he bent over. That's when he felt the pain in his lower back. Strain - low back.
