Reporting occupational illness of employee. Employee had previous injury on 5/22/02 to his left knee and was away from work until 5/17/04. He was off once more for approx. (6) months in 2005 due to problems once again with his left knee. Employee started missing work once again on 2/19/07 due to problems with left knee.
