Narrative

EE was installing a cable bolt in intersection of #3 entry at spad #6273. While pushing the bolt up, EE started to rotate bolt. After pushing bolt up approx 5', the bolt became frail and bowed out while spinning, striking EE on the left arm. Diagnosed with fracture at RGH Hospital. EE has a cast on his arm.
