Narrative

A roof fall was found by MSHA Inspector and mine examiner while examining the A-3 Mains, 1st Parallel Panel, 3 Blocks outby Spad # 2450. The roof was approx 10' high x 70' long x 20' wide. The area was supported with 4' resin bolts. This area was 7000 ft outby the face.
