Narrative

Employee states as he was preparing to remove wire mesh and loose coal from walkway, a chunk of coal fell from rib. As he moved away from the falling material, the wire mesh and loose coal under foot caused him to fall backwards and he struck his head against the side of the head drive. He received a cut on the back of his head which required 5 stitches.
