Narrative

Employee stated that he was walking thru crosscut 2-3 toward a sump to see if the 480v sump pump was working. While doing so he caught the toe of his right foot on a yellow hook rope that was hooked to piece of old curtain laying on the mine floor. He fell straight forward to the mine floor, hyperextending his right ankle.
