Employee was pouring cement from pump truck on second floor. Cement had hardened inside the line, then discharged with great force throwing employee backwards onto floor causing employee to hit elbow and back on rebar that covered the floor. He was hired thru a temporary agency which was found not to have a contractor's id number to file 7000-1.
