Employee was packing a piece of belt structure. He stepped over a concrete block and felt a pain in his left leg and back. Employee continued to work and continued having pain. He scheduled an MRI on 4/19/07. The MRI was positive. He began missing time on 4/25/07 due to surgery on lower back.
