Narrative

On 4/23/07, an unintentional roof fall occurred on the 1st North Panel off the 2nd West Sub Main off the 4th North Sub Main. This roof fall was supported with 6 point anchor roof bolts. The roof fall measured 30' long X 13' wide X 2' high. The roof fall will be cleaned up.
