## Narrative

Employee was using sledge hammer to drive in bars on slide of 1E clinker tank prior to calibration, when hammer slipped off end of bar, flew from his hands becoming wedged between clinker hut and handrail.  Employee fell onto end of hammer handle when he lost his balance.
