A roof fall occurred in the 3 South Panel #3 entry starting inby cross-cut #13 and extending through the intersection of cross-cut #14 measuring approximately 55ft. x 18ft. x 9ft. The area was supported with 5 foot fully grouted roof bolts and 9 ft. mega bolts. An undetected slip was determined to be the cause of the fall. This area was mined in April 2007.
