## Narrative

Roof bolter was installing his second roof bolt.  Employee then inserted the roof bolt into the hole and was holding the bolt in position to bring up the drill motor carriage to accept the roof bolt.  He inadvertently grabbed the wrong handle, which activated the drill steel centralizing mechanism.  Centralizer clamps closed on his left index finger.
