Narrative

Mechanic climbed up & was standing on a 3 1/2"" angle iron. He was putting a chocker around a counterweight in order to lift it up & change the take up roller. After chocker was in place, he was attempting to get down, when he lost his footing, fell & landed on his back on the grating below.
