Narrative

Installing an H beam on 2 West Transfer. Employee was on off side of transfer with a come-along & ladder. The rest of the crew was on Cool Hill #3 belt, with backs turned away, lifting H Beam by hand. Injured was to pick his side up with come-along. Slipped off the 3rd step of the ladder and fell to ground.
