## Narrative

Bolting machine broke down because the drill head became fouled. As the EE was attempting to free the head, the bolts in the bottom of the drill feed jack broke, allowing the lower part of the jack to become free as the EE was attempting to free the head up and he placed his left leg under the head of bolter. It suddenly became free and fell approx 36"", catching his left leg.
