A roof fall was occurred on the 2B panel in the return at crosscut 4 from entry 2 to entry 3 and a half. The cause of the fall was weak laminated shale. The area had been initally supported with 6 and 8 foot bolts as it was mined. The fall blocked the weekly examiner's route of travel. The return was re-routed around the fall area.
