Additional ground support was being installed in an area that was in need of it. Two 8' split set bolts had been installed and more holes being drilled. Small material started ""dribbling"" at the brow so they backed off a safe distance to observe it. After about five minutes the brow & more material fell exposing two 8' swellex bolts that appeard to have been installed at an angle.
