Narrative

Their was a roof fall at brk 13 of 9 f. belt at spad #41473 in #6 entry. Fall will be cleaned up. Fall was approx. 20'x20'x5' thick top was bolted with 5' torque tenion. bolts, excape way was not blocked. Strate was shell.
