## Narrative

Employee was installing a spacer between a kiln gas pipe and the flex pipe. He was removing a 7/8"" bolt from the flange because the bolt hole did not line up to the bolt. Employee was pulling the bolt and the bolt let go coming out hitting his right front tooth, breaking the tooth.
