Narrative

Employee was attempting to enter the discharge end of the rotary breaker via the use of an aluminum platform used to span the opening of the breaker rock discharge chute. A weld broke on the stabilizer clip on the platform causing the platform and the employee to fall through the breaker rock chute to the rock holding area at ground level.
