Narrative

The employee was working on the secondary plant discharge line removing a bolted cover on a check valve. He was positioning a wrench on a nut when co-worker activated a 3/4"" drive impact wrench on the bolt head. This made the wrench spin with the nut and EE's finger was caught in a pinch point between the wrench and the pipe flange.
