Narrative

Employee was bolting a straight in entry #1 44 + 20 in Unit #1. He was installing the middle bolt when a rock fell out and struck him on the back of the neck and between the shoulders. The rock was 3 ft long, 26 in wide, and 3 1/2 in thick. The rock broke up when it struck the employee.
