## Narrative

Employee was climbing up the steps of D-30-D ore truck when he slipped on the ice. He fell bumping his left elbow and right knee. As he was falling he grabbed the hand rail and pulled his rig ht shoulder. On 6/19/2007 the employee first saw a Dr about shoulder pain and on 7/3/2007 was put on lifting restrictions. No lost time yet.
