The roof fll in #3 Entry at 42 Blk. North Mains Return. Idle cover approx. 365' Top gray shale. Area was bolted with 6' fully grouted resin bolts and 12' cable bolts used as supplemental support and 16 x 16 plates. Fall was discovered on weekly exam.
