The roof fell in #4 intersection at 40 blk north mains return idle cover of aprox 365', top gray shale, area bolted with 6' fully grouted resin bolts, 12' cable bolts used as supplemental support and 16 + 16 roof mats. Fall was discovered on weekly exam.
