The roof fell in #5 & #6 intake & X5-6 at 43bl. Worth main return MM V Idle. Cover of approx 365', top Gray Shale. Area bolted with 6' fully grouted resin bolts and hole board & 12' cable bolts used as supplemental support with 16"" X 18"" mountain mats. Fall was discoved on weekly exam.
