A foreman performing a pre-shift examination for the day shift on 7/5/07 found a roof fall as he was returning to the bottom from #2 section. The fall was located adjacent to the 5 West roadway between entries 5 and 6 at crosscut 40. Travel was halted in the area until after cribs were set.
