Narrative

Employee was assisting setting conveyor belt tail piece. Employees tried to pry loose the stuck belt, the come-a-longs holding the tail piece loosened and the tail piece fell down on the employee's right foot. Compliance with rules/regs not a factor. Protective equipment not a factor. Mining equipment/sytems not a factor. Miner proficency a factor.
