Narrative

Employee was changing the teeth on the shovel bucket, teeth keeper pins had been removed. He was using a 10 pound sledge hammer to drive the old teeth off of the adapters. When he hit one of the teeth with the sledge hammer a small metal fragment (approximately 7 mm by 4 mm paper thin) chipped off of the tooth and lodged in his left forearm about six inches below the elbow.
