Narrative

Employee went up a ladder to repair a lube oil on GL, when the wrench he was using fell and he yelled to the employee holding the ladder to watch out. Employee holding the ladder stepped out of the way of the wrench and the ladder slid out from underneath employee, causing him to fall 20 feet.
