Narrative

Injured was kneeling on his right knee at intersection, 4 rows back from face operating continuous miner, taking 1st cut of of 1 left gob eye on 004 Section. A rock fell out from between bolts striking him on his back & right hip, resulting in fracture to rightt hip. Rock measured approx. 71"" long x 36"" wide x 1/4"" to 5"" thick & mining height was approx. 82"".
