Narrative

While pulling fueling hose from reel on back of fuel truck to refuel equipment between shifts, he turned (twisted) to pull more hose from reel & felt sharp pain in lower back. Refused treatment at time of reporting incident. Went to Nurse Practitioner 7/19/07 who ordered MRI. Doctor who read MRI 7/20/07 stated he had a herniated disc & was to stay off work.
