Narrative

A roof fall occurred in the 1 north panel, crosscut #9, #5 entry. The fall measured approximately 30'x18'x3 1/2' - 4'. The area was supported with 30"" fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.
