Narrative

Employee states that the #16 shovel struck the dove tail of the #518 haul truck while being loaded. The validity of this claim was questioned, a 7000-1 was not submitted. We received notification from our insurance carrier that this claim was determined to be compensable; therefore we are now submitting a 7000-1.
