Narrative

Employee states she was doing a walk-around on her haul truck and slipped in the mud. The validity of this claim was questioned, a 7000-1 was not submitted. We received notification from our insurance carrier on 8/22/2007 that this claim was determined to be compensable; therefore we are now submitting a 7000-1.
