Narrative

The employee states that he was hauling supplies to the roof drills. He was carrying the rope bolts on top of the scoop and was passing through a set of fly pads when one of the rope bolts hooked on the fly pads and sprung back striking him on his right forearm causing a laceration that required stitches.
