Narrative

Employee stated that he was roofbolting in a rock fall area where the roof was higher than usual. As he looked up to watch his drill steel, a small piece of rock fell from the roof and struck him in the mouth. A front tooth was broken off and he had a laceration to his upper lip that required 3 stitches.
