Narrative

Employee reported: Operating roof bolter in face area, stepped on damp piece of draw rock and slipped. When he slipped, twisted his left knee. Reported to his foreman pain to the knee on date of injury. Started missing scheduled work days on 9/12/2007. Became a reportable injury on that date.
